Welcome. The first step is to fill out the intake questionnaire at the link below (you will be redirected to Simple Practice, the secure site used for patient portal). Please include a brief description of why you are seeking care and how you found me. We will then find a time for a free 15 minute phone call to determine if we are a good fit for an assessment appointment. 

If we both decide to proceed, then we will schedule an assessment appointment. Please note that that the first appointment is an assessment only, and does not guarantee the establishment of an ongoing patient-doctor relationship or future care. I want to ensure you are in the right treatment setting, and it is possible that I may not be the right fit.
