New Patients
I am glad you are reaching out for care. The first step is to fill out the intake questionnaire at the link below (you will be redirected to Simple Practice, the secure site used for my practice). You may also leave me a voicemail if you prefer. Once I receive your questionnaire or voice message, I will follow up with a brief free phone call to determine if we are a good fit for an assessment appointment.
If we both decide to proceed, then we will schedule an assessment appointment. Please note that that the first appointment is an assessment only, and does not guarantee the establishment of an ongoing patient-doctor relationship or future care. I want to ensure you are in the right treatment setting, and it is possible that I may not be the right fit.
Please do not use this form for emergencies. Instead, please call 911 or go to your nearest hospital.

Please use the link below for the patient portal on Simple Practice. This is where you can access information related to your care, complete forms, send messages, view appointment schedules, see billing information and update your personal/contact information.
